We have an forms which teachers feed back on which we then use for the reviews. The easiest way we work it is to set 2 targets at whole school and then a personal development target set by the support assistant. So for example last year we had: Use of questioning and supporting without doing as targets. Then they have their own target. Over the years under this system, I have now been able to pass the reviews to year leaders to do.
