For early years my most important outsourced assessement is OT – for gross, fine and graphomotor issues as well, and very importantly at this age, sensory processing/integrating issues. Speech and language I would go for after 4 years.
I keep a running document called Noticing a child. On this I write notes about every child the teacher or TA ‘notices’ . I add to each child’s box as we go along. Some settle and I no longer monitor them. Soem’s profile changes and we move on to an assessment – in which case I write out an Individual Learning Plan describing the child’s profile plus the measures we are taking to support the child.
I also keep a register – sometimes I remove a child from the register when s/he is fititng in with the flow of the curriculum.
