This a the form I used for assessments for all students.

Devised by me – no copywrite involved – appropriate for any age group

I used this to find out exactly what is going on in my classroom, to monitor a single student who is causing concern in some way – or a group of students. I also used it as a tool for mentoring staff and as part of the appraisal process. It is particularly useful because it provides evidence and it is a way that you can demonstrate to students exactly what they need to change in order to improve their working habits. It will also tell you who you should be heaping praise on too! It all depends how you use it and what you choose to record.
