    As part of my ADHD referrals I have to send in completed SNAP forms.
    If the child does not score more than 13 in inattention and hyperactivity, they deny the referral. I’ve asked the parent and teacher to complete page 1 before to show them they actually didn’t score them that highly and therefore a referral is not going to be accepted.
    I’ve uploaded the file below. Page 1 is the scoring and Page 2 shows the criteria. I keep Page 2 to myself so parents don’t inflate data to meet the threshold.

    If the child is very different at home and school, maybe just inform parents of strategies that work well at school eg.visual timetable, routine, clear boundaries etc.
