That’s what I was about to suggest. There are lots of similar forms on line. It’s really interesting to see if the behaviour at home is consistent with school. Then if it’s not, a referral from school wouldn’t really be appropriate. But maybe establishing some joint home/school strategies would be the way forward instead – so the child gets more consistent boundaries etc. It sounds as if the strategies that you are using at school are working – so maybe suggest using them at home too.
