Hi. Section K of the EHCP should have all the details of those involved. I always meet with the parents as soon as the pupil starts at school. Additionally the local authority SEN Caseworker or EHC Coordinator responsible for the plan can help if you’re not sure. I made a table of all my pupils with EHCPs, when their plan was finalised, when the annual review was due (6 month review for EYFS or 12 months thereafter), who the professionals involved are etc. 6 weeks before the review date I would send a few suggested dates to parents, teachers and professionals and agree on one. Anyone would couldn’t make it I’d ask for reports and I’d present them. Hope that helps though I’m sure you have a great system already!!! I think regular communication with parents helped me plus the IEP review meetings with teachers flagged stuff up.
