Some referral processes make you complete the SNAP IV checklist. This tells you what threshold/score they need to meet on the second page:

It’s also a great tool for showing parents their child does NOT show symptoms of ADHD and saves the faff of a referral.

But I know most places won’t let you refer until they are 6 or 7 years old.
