Thank you all for your replies!I like the the idea of promoting the digital referral form on a weekly basis and then discuss it with the HoYs.

We have a digital referral form, which teachers do not always complete and end up raising their conern verbally or via email.
Some teachers raise concerns with Head of Years but then I find it not to be an effective way as HoYs cannot always filter whether there might be any underlying learning difficulties.
Once a concern is raised we ask for all subject teachers to write their feedback via email and we take it from there.
