I use a SEND Referral Form which has three questions. 1. What are your specific concerns? 2. What specific high-quality teaching and learning strategies have been implemented to support the student? 3. What has been the impact of your support/intervention?
If staff send me a general email or have a chat in the corridor about a student the first thing I ask them to do is to complete the SEND Referral Form and then I act on it. It works really well and keeps a track on referrals.
Most importantly, teachers have to think about their role as teachers of SEND. Hope this helps. Pat
