I converted the questionnaire we previously used into a Microsoft Form. You can export the responses to an excel spreadsheet to collate the responses.
Teachers prefer it because they don’t need to download then upload to respond – much higher response rate. Try to make as many questions on the form skippable as you can – it saves people time wasted to unnecessary deliberation.
