I’m not aware of the official paperwork that was completed but as the medical plan was only finalised a day before the exams started, I can’t imagine anything was applied for.

My daughter’s Specialist Paediatric Diabetes Nurse was the person, who stated the adjustments required. She and I met with school and all relevant information was written into her plan.
