If it is possible could you orchistrate mum needing to use the toilet next time she’s in the building I’m guessing this is a student toilet but other students would be kept out for this charade – obviously. If the student sees mum, or a trusted adult, use it then it may lesson any anxiety. Also, have you checked for spider phobias, cleanliness, noise anxiety?
