    No rules about IEPs – as long as you have followed ‘assess, plan, do, review’ you can address that need in whatever way you think best – always thinking of ‘the right provision at the right time’. Not sure if you work in primary or secondary, but in secondary, where students have 18 ish teachers, I do not believe IEPs are at all effective. The key is to have very accessible information and strategies for each student that teachers are required to know and work to – and you find a way of checking with e.g. learning walks.
