I would include information on:

PSED and communication skills

Self regulation skills & behaviour, ability to follow instructions / compliance

Any unusual patterns of development (eg can recite the alphabet, but doesn’t respond to own name)

Write a “person on a page” description so the clinician gets an idea of how this child interacts with adults and peers, if they use imaginative play, self care skills etc
