It might be worth contacting your local Learning Behaviour Advisory Team (LBAT). They can offer advise and support in this area. I have recently just spoken to them about a child who has just started in KS2 but is working a pre-key stage levels (possibly even EYFS in some areas). They have been really helpful with guidance on what to do next as intervention and support.
