When I do a transition these are my steps:
1. Reach out/Meet new Sendco
2. Get SENDCO to send a transition story for YP (with photos, class teacher areas in the school, school timetable etc)
3. Take a copy of whole school file (ILP, PLP, provision maps etc) and I drop it by hand or recorded delivery (dependent on location, or use egress if all parties agree and the school can print off themselves).
4. Pre-teach leaving school and a new setting.
So you can ZIP or handover a hard copy.
